บทคัดยอ
โรคเรื้อรังเปนปญหาสําคัญของสาธารณสุขทั่วโลกซึ่งรวมทั้งประเทศไทย แนวคิดนวตก
รรมการดูแลโรคเรื้อรังขององคการอนามัยโลก (WHO’s Innovative Care for Chronic
Conditions Framework) เปนรูปแบบการดูแลที่องคการอนามัยโลกพัฒนาขึ้นเพื่อใหเหมาะสม

กับบริบทของแตละพื้นที่แตละประเทศ ผานการดูแล 3 ระดับ คือ ระดับของผูปวยและครอบครัว
ระดับระบบบริการสุขภาพและชุมชน และระดับนโยบาย ผลการประยุกตใชในพื้นที่ภาคเหนือ
พบวาพยาบาลวิชาชีพในระบบบริการสุขภาพระดับปฐมภูมิไดแสดงบทบาทในฐานะผูพิทักษ
ผลประโยชน ผูใหความรู ผูปฏิบัติใหบริการและผูประสานงาน แสดงใหเห็นวาพยาบาลวิชาชีพใน

ระบบบริการสุขภาพระดับปฐมภูมิเนนการบริการเชิงรุกเปนสําคัญ

Abstract
Chronic diseases are a major problem of public health around the world,

including Thailand. The WHO’s Innovative Care for Chronic Conditions Framework
(ICCC) is a model of care that the World Health Organization developed to suit the
context of various countries and to achieve better health outcomes for chronic
disease patients. This model consists 3 levels of care management, including

individuals and families, health service organization and community, and policy
level. The implementation of WHO’s Innovative Care for Chronic Conditions
